ORGANIZATIONAL BACKGROUND

PCC Community Wellness Center (PCC) is a not-for-profit comprehensive community-based health center located on the boarder of Chicago and Oak Park, Illinois. The Center was founded in April 1980, and operated as part of West Suburban Hospital. In June 1993, PCC was incorporated and became a separate entity from the Hospital. On July 13, 1993, the Center received 501 (c), (3) designation. PCC received approval as a federally qualified health center on January 14, 1994. In February 1997, it opened a second Center located in the Austin community of Chicago. The Health Center is governed by an independent governing board.


SERVICES PROVIDED

PCC is committed to providing quality care and provides a full spectrum of comprehensive health care services such as pediatric health care, women's heal care including family planning and prenatal care, and men's health care.  The Center also provides on-site support counseling, disease screening, immunizations, school physicals, WIC screening, and outreach services.  Diagnostic laboratory, radiology services, emergency services, and in-patient care is provided at West Suburban Hospital Medical Center located just one block from the Center.
